For us to assess your application, please answer each of the following seven questions. We require supporting evidence for each 'Yes' answer, so please select 'upload a file' to upload a document. All information provided will be treated as confidential. For further help click here

1. Do you use PLANITHERM? Yes No
If yes can you please upload an invoice or delivery note from your PLANITHERM supplier
Who is your PLANITHERM supplier?
2. Do you have any third party accreditation - for example from FENSA, Certass or GGF? Yes No If yes please provide evidence

3. Do you have an insurance backed guarantee scheme? Yes No If yes please provide evidenceThis is a policy which is issued to the owner of the property by an insurance company to underwrite an original guarantee provided by the installer.

4. Do you have a code of practice in place? Yes No If yes please provide evidenceThis is a set of guidelines and regulations which must be followed by all members of your company in order to ensure the quality of the service provided to the customer.
